Context matters.

This is true for literally every hand gesture. In most of the world, if someone gives you a thumbs up, it's a sign of approval... But in other parts of the world they are effectively flipping you off.

When most people flash the "okay" hand sign, they just mean "okay". When Richard Spencer does it, or some white cops after a shooting, I think it's fair to question what they mean.
